Coordination of care can be modified through change management which will impacts the patient’s experience. Fiorio, Gorli, and Verzillo report that transitions from an antiquated model of care to more modern patient-centered care requires change management in hospitals and other health care service facilities. Policy and procedural changes can be shaped by the leadership of hospitals and health care facilities to carry out care with more of a patient-centered focus. All the decisions shouldn’t come from the top down though, organizations should be open to listen to the input of subordinates. With these strategies for change management in mind, patient-centered care is more likely to be achieved and will thus improve patient experience
